1.  On the  drop down box labeled Name on Shirt # 1:

          Enter your first and last name and then your size

2.  Drop to the Additional Info Box ( it's after the LOGO Placement Info Box) and enter:

         This is a Puyallup Hawg Hunters Team Shirt/Same as Tom Melowitz

3.   Enter your personal info and then submit order
