1.  Once there click on the first drop down box with the label of:
      Level 3 Shirt  Design/Color

           Select:  Design 1 Color: BLUE (Banded/Zip)

2.  On the second drop down box labeled Name on Shirt # 1:

          Enter your first and last name and then your size

3.  Drop to the Additional Info Box ( it's after the LOGO Placement Info Box) and enter:

         This is a Puyallup Hawg Hunters Team Shirt/Same as Tom Melowitz

4.   Enter your personal info and then submit order
